The "Take Charge!" Program

 The "Take Charge!" Diabetes Self-Management Program at Duluth Diabetes Center is all about empowering patients and their loved ones through education. It puts patients in the driver's seat for managing their diabetes. It's a tool that encourages patients to not just live but to thrive with their condition.

Individualized care and instruction is key to our approach. Each patient receives personal treatment that emphasizes how they can help themselves. "Take Charge!" has been recognized by the American Diabetes Association (ADA) as meeting national standards of diabetes self-management education since 1991.

Follow-up care, based on individual self-management needs, is available through office visits, phone contact, e-mail or fax.
