This expertise, delivered with a caring touch, helps our patients maintain their ability to get around and their quality of life overall.

Our patients don't have to come to us to get care — we go to where they are

Besides being much easier for our patients, our practice of visiting them where they are allows for timely evaluations and treatment. And that reduces visits to the emergency room and hospitalization. For patients in rehabilitation, our coming to them creates more time for physical and occupational therapy appointments, since there are fewer trips to the clinic.

Helping to manage multiple conditions

As people age, it becomes more and more important to assist them in managing chronic and multiple diseases and conditions, as well as the accompanying medications. We interact closely with other specialty departments to coordinate care. We also work jointly with hospice and palliative care to keep patients comfortable and cared for.

And if one of our patients develops a fever at 2 a.m., they don't need to wait until morning for our response. We are on call 24 hours a day, seven days a week.
