Health Care Home

Health Care Home is an ideal care model for the primary care of children with special health care needs.

As defined by the American Academy of Pediatrics, this model provides accessible, family-centered, comprehensive, continuous, coordinated, compassionate and culturally effective care for which the healthcare team and the family share responsibility.


Since its inception in 2007, the Duluth Children’s Health Care Home team has made great strides in helping to simplify healthcare access for children with special health care needs. Some examples include:

  • Creation of a direct hospital admission process for specified patients to avoid emergency department time and costs
  • Parent collaboration with neurology, palliative care and other subspecialists for comprehensive care planning.
  • Enhanced scheduling, care coordination and parking access for families who need it most.

Each child enrolled in Health Care Home is given a care plan that outlines his or her medical history. It stays with the child, so it can be brought to appointments and can be used in case of an emergency.

Because the Health Care Home model offers families better access to their doctors and to the healthcare system, its goals are to keep children healthier and reduce families’ worries about getting the care they need.
